1. Service Coordination (SC)
Service Coordination (SC) is at the heart of what we do. It’s more than just connecting people to services it’s about ensuring individuals and families receive the right care, at the right time, with the right support. Our Service Coordinators work side by side with clients to understand their goals, challenges, and personal needs, then create a tailored plan that makes navigating healthcare and community services simple and stress-free.
From scheduling medical appointments and linking with housing or financial assistance programs, to coordinating therapies, education, and disability support, we handle the details so clients can focus on living their lives. We also advocate strongly on behalf of our clients, making sure their voices are heard and their rights respected in every setting.

2. Community Integration Counseling (CIC)

3. Independent Living Skills Training (ILST)

ILST is tailored to each person’s unique goals and abilities. Whether someone is learning how to use public transportation, organize their daily routines, or manage medications safely, our trainers provide hands-on guidance, encouragement, and ongoing support.

4. Positive Behavioral Interventions and Supports (PBIS)
